Representation Agreement

This is a document wherein you designate and name your “health care” representative EVEN when it’s a spouse and children.  This is a document which contains end of life authority for your representatives.   This document can become critical for allowing your representatives to be your advocate and part of your ongoing long-term care planning.

At dks Legal & Notary Services we recommend that you prepare a Health Care Directive at the same time you have your Power of Attorney prepared to assist your representatives and Health care professionals, which we will provide at the time of the appointment.

These documents can be very emotional.  We recommend you download our “intake” form on this page, read completely through one time and then complete as best you can then call into the office to set a time for your appointment.

ID Requirements

You will be asked to provide two pieces of identification, one of which must be government-issued such as a Driver’s Licence, BC Identification, Passport or Permanent Resident Card. It is critical that the names must match on the identification AND your documents. Please call prior to your appointment if this is not the case. Note: a combined Drivers’ Licence and Medical card only count as one piece of identification.
